Email & E-Signatures
These policies apply to all e-mail and e-signatures sent by Diversified Health Services, Developmental Disabilities Management Services, Diversified Health Care, and their affiliated entities (“Company”) and are the conditions under which the Company authorizes their officers, members, employees, and agents to use electronic signatures.
All electronic communications, including any electronic signatures or identifiers of the sender, are being transmitted solely for informational purposes. By making the electronic communication, the sender and the Company do not intend to agree, bind, or otherwise consent to any matter for any purpose whatsoever, and do not intend the electronic communication, or any related or associated electronic media or data, to constitute, or to be deemed to constitute, an electronic signature under any law authorizing the use of electronic signatures. The Company utilizes electronic signatures only when accompanied by a statement to the effect the sender specifically and expressly intends the electronic signature or other identifier of the sender as being transmitted as an electronic signature under or by a law authorizing electronic signatures.
Emails sent from Company are confidential and intended solely for the use of the individual or entity to which they are addressed. If you received a communication in error, please advise the sender by reply email and/or contact our office at 901-692-5555 and destroy the original and all copies of the message. Please do not send the Company or its officers, members, employees, or agents any protected health information or confidential or sensitive communication without first speaking with and obtaining prior authorization from one of our officers, members, employees, or agents to send such communications.

Please be aware that email communication can be intercepted in transmission or misdirected. Your use of email to communicate protected health information to us indicates you acknowledge and accept the possible risks associated with such communication. Please consider communicating any sensitive information by telephone, fax or mail. If you do not wish to have your information sent by email, please contact the sender immediately.
No Patient Relationship
Transmission, use, and receipt of the information on our website and communications sent via email or other methods do not create, and are not intended to create, a patient relationship between you and the Company or its officers, members, employees, or agents.
